ABOUT US
Direct Primary Care is a membership-based model of care that eliminates insurance interference with care. At Uplift Family Medicine, I provide patients with unlimited visits throughout the year without additional copays/deductibles. Appointments are 30 minutes -1hour in most cases, depending on need. There are no wait times. Patients can reach me, directly, by phone/text/email 24 hours a day. My goal is to meet everyone where they are in life to provide the best care possible. I believe that quality care should not be dictated by what insurance companies determine is necessary or not.
We believe in being available when others are not. This eliminates unnecessary trips to the Emergency Department because you are able to get an appointment with me same-day or next day. Contact us to find out more, or to schedule a complimentary 15-minute meet and greet.

About Dr. Augustyniak
I grew up in a small town in North Central Louisiana. I was a non-traditional student. At the time I met my husband, I was a nanny and a massage therapist. Until my children were both in school, I was a stay-at-home mom. Once they were in school, I decided I wanted and needed more. I decided to return to school and pursue my medical degree. I attended Louisiana Tech University, and received my Bachelor's degree in Biology. Received my medical degree from Lake Erie College of Osteopathic Medicine, and completed residency in Family Medicine at Texoma Medical Center in Denison.
I fell in love with the Texoma community from my first visit here. I moved here in 2017 and decided to make this my home. This has to be one of the friendliest communities I have ever been in.
My unique work experiences allow me to provide comprehensive family care, including urgent care I have spent the last three years in a hybrid position as emergency medicine physician and hospitalist, as well as continued as adjunct faculty/community preceptor for Texoma Medical Center Family Medicine residents.
I have a passion for preventative medicine, and want to be able to provide care for patients in a way that really makes a difference. I do not believe that comprehensive care can be done in a 15 (or less) minute appointment, especially when there is a 2 week or 2 month wait for an appointment. This is why I have chosen to open Uplift Family Medicine.
In my spare time, I enjoy spending time with my children (now 20 and 17 years old), swimming, and reading.
